A new patient with no family in the area who drives occasionally for Door Dash and is living in her car or with friends; she was living in an old trailer that leaks and has mold she can’t breathe well inside during the cold months. We gave her referrals for help with accessing food, clothes, heat and showers, trailer repair, and shelters. We provided her a free hygiene kit. She was diagnosed with hypertension and was referred to a 1:1 class that day for hypertension. During that class she was given a free automated blood pressure cuff. Her blood was drawn and tested for free. She scheduled a follow up appointment with medical as well as an appointment with spiritual care. She left in tears for all the care and love she received.

A 36-year-old patient who has a 10-year-old son was experiencing weakness, fatigue and lack of motivation, but did not have medical insurance to seek the care she needed. On her first visit, we conducted lab testing and learned that she was diabetic. We referred her to our Diabetes Program and she has made great progress in managing her blood sugar levels. We also provided dental care and referred her for an eye exam and OB/GYN.
